TNS believes in promoting good health by developing and maintaining healthy lifestyles. Social and recreation supports are key for you to achieve healthier and active lifestyle. We help you to develop ways to achieve a healthy physical and mental well-being. We can assist you in meeting new people, taking part in a group activities, assist you in coordinating your plans, and assist you in improving your self-esteem and social skills.

Our Services

Transform NDIS Support believes that EVERYONE has the right to access support to live a vibrant and independent life regardless of race, religion, language spoken, country of origin and any form of disability. We work with community groups, government agencies, health providers and other stakeholders to ensure mental, physical, emotional, and social needs of all participants are met to the highest standards.

We will carry out face to face assessments with you and your representative to understand your strengths, needs, and goals to design support services that will assist you in maintaining and improving your lifestyle, independence, and desired goals.

Our support workers are well adept in supporting you with your activities of daily living, community/social engagement and capability building.

Transform NDIS Support has highly skilled support workers who have been trained in delivering high intensity care to participants in their place of residence. We have support workers who are trained in diabetes management, complex bowel management, ventilation management to name a few.
We offer support in the comfort of your residence to assist you with your activities of daily living, medication management and relevant supports needed to achieve a good quality life.
We provide evidence-based care to people with complex health conditions under our Community Nursing Services with qualified ENs or RNs.
We have purpose-built accommodations to meet the needs of individuals that require homes that are specially designed to meet their needs.
We understand that trauma can lead to various behavioural challenges. We work in collaboration with a positive behaviour therapist and/or other service providers to develop an appropriate plan. We listen, plan and work with you to navigate through behaviours and build capacity to cope with day to day.
We work collaboratively with you to meet desired outcomes to achieve involvement and enjoyment in the community you connect with.

High Intensity Care

Enquiry

Referral received and reviewed by the multidisciplinary team. Response from TNS within 24 hours.

Consent to access plans/documents

Acquisition of consent to access relevant documents and data for a more accurate assessment of support required.

Primary assessment

A face-to-face meeting (if applicable) to conduct initial assessment of your needs. We will review your plan, goals etc with you and/or your representative. TNS team will then ensure that care needs are identified and a structure of the services to be provided are drafted.

Engagement in support plan

Proposed service agreement will be discussed with you and other stakeholders. Ones all agree service agreement is signed.

TNS will work with other stakeholders allocate resources required by a participant are in place before commencing the service.

Service Implementation

TNS will commence service implementation in agreeance with participant and other stakeholders.

Service Review

Regular and frequent collaborative meeting with participants and other stakeholders to ensure and review tracking of expected outcomes.

We have lived experiences

Our team comes from diverse backgrounds, but we all have something in common, we all have lived experience in one form or another. We are aware of how important it is for our family members to have a well-balanced life and how achieving this can be a battle. We understand the fears of families to ensure a family member is safe and cared for when they are not present. Our processes include regular service reviews to ensure that supports are evolving according to needs and capabilities gained.

Meet our Team

The Transform NDIS team are composed of individuals that are warm, friendly and purpose driven. We are headed by a multidisciplinary team of FRACGP Doctor, Quality and Compliance, Registered Nurse, Social Worker, Allied Health Assistant and others. We all work in a collaborative manner to provide holistic services to our participants.

Pricing updates – announcement 22 June 2022

Price inflation of 9% to address cost pressure. Hence, from 1 July 2022 NDIS has increased price limits for services delivered by disability support workers by 9% to mitigate the cost pressures on providers, including:

  • Costs associated with managing the COVID-19 pandemic
  • NDIS quality and safeguards regulatory requirements
  • Fair Work Commission’s recent wage decision
  • Superannuation guarantee charge increase.

Making NDIS payments faster and safer

The NDIA has recently concluded a Request for Tender (RFT) to build a new Claims at Point of Support (C-POS) channel, giving participants and providers a faster, more secure way to make their NDIS claims.

The Commonwealth Bank of Australia was successful in this process and will partner with the NDIA to create a better option for managing claims, without the need for participants to pay up front using their own funds.

Continued Covid support for NDIS participants

Minister for the NDIS, the Hon. Bill Shorten, wants to assure participants they will continue to be supported with proportionate and targeted measures that reflect the significant health risks many people with disability continue to face due to COVID-19.

The National Disability Insurance Agency (NDIA) has updated its COVID-19 measures to ensure participants and providers continue to be supported.
